Please print the USEF agreement on the back of your entry form.
For your entry to be complete we will need both documents filled out completely and signed.

Also please note the following correction to our showbill:
Regarding classes #20 and #61: tests of choice include both the USEF four year old test and the FEI four year old test.
Please specify on your entry form which test you are riding.
